LCWC Facility Guidelines



Records and Procedures:

1. Membership Suspension: In the case of a medical problem, your partnership may be put on hold for the time of the illness but not to exceed 6 months. You will need a slip from your physician to stop and start your partnership. If you have an auto debit account your payments will still be deducted as scheduled, and the time of your suspension will be added to the end of your original expiration date.

2. Early Termination: Early termination fee of 65% of the balance due will be assessed if the investment form contract is broken for the following reasons only (proof may need to be provided): a) Loss of a job b) Moving out of the area c) Medical necessity. If the investment form contract is broken for any other reason than that listed above, the balance owed at the time of termination will be the amount due.

3. Defaulted Accounts: Any accounts that are defaulted will be sent two notices of amount due. A 10% late fee is charged on the second notice. After the second notice the LCWC reserves the right to pursue collections.



Children’s Guidelines:

1. 17 and Under: Children under 18 years of age must have written parental consent before they can begin exercising.

2. 8-13 Years: Children age 8 through 13 can participate with an adult member. Such children must be within the “arms reach” of the adult member at all times in all areas of the LCWC. In addition, a separate liability form needs to be signed by a parent or guardian.



Facility Etiquette:

1. Check-in: All partners using the LCWC must present their partnership card at the front desk. The LCWC reserves the right to inquire into the identity and partnership status of any individual. Upon completion of using the facility, the partners must either take their partnership card with them or file it back in the partner card box.

2. Health Screening Form: Partners must complete a health screening form (PAR-Q) and sign an investment form provided by LCWC before or at their initial visit.

3. Medical Release: LCWC’s personnel have the right to deny exercise privileges based on partner’s health questionnaire until physician’s written approval is obtained.

4. Proper Attire: All members will be expected to wear appropriate attire when using the facility. Shoes and shirts are required at all times in all areas outside the locker rooms and pool areas. We ask that athletic shoes which are worn outside not be worn on the exercise floor. Sandals or open toe shoes are not allowed except in pool areas.

5. Valuables: Purses, wallets and other personal items should be locked in lockers. Locks can be rented at the front desk. LCWC will not be responsible for any lost or stolen items.

6. Towels: Partners are encouraged to bring their own towels. Towels are available to rent at the LCWC front desk. Washcloths are free to all partners and should be used to wipe down equipment after exercising.

7. Lockers: Lockers that are not rented out to partners must be emptied on a daily basis. Locks left on lockers overnight will be cut off and the contents of the locker will be held behind the front desk. Partners must provide their own locks and are encouraged to use them when using the lockers. Locks are available at the front desk for a fee.

8. Food and Drink: Food and open drinks are not allowed in the workout or pool areas with the exception of water.

9. Use of Pool and Hot Tub:

a. Members are required to shower before entering the pool and hot tub.
b. Children under the age of 12 years may use the pool only under the direct supervision of   an  adult.
c. Arm flotation devices are not permitted. Please bring a life jacket if necessary.
d. Proper swimwear is required. This includes one-piece tank or conservative two-piece style suits for females and competition or trunk style swim suits for men. Thong style swimsuits and cut-off shorts are not permitted.
e. Running, jumping in or diving in the pool are not permitted.
f. Street shoes are not permitted on the pool deck.
g. All children who are not toilet trained will need to wear swimming diapers. If you forget to bring swimming diapers, they are available in the Wellness Store.
h. It is not recommended that children use the hot tub. Blood pressure and body temperature rise quickly in smaller bodies, which could be potentially harmful to the child.
i. Individuals with any cardiac or pulmonary disorders should consult their physician before using the hot tub.

j. No one can use the pool or hot tub during a scheduled class or physical therapy.

10. Assumption of Risk: The use of the LCWC shall be undertaken by a partner or guest at his/her own risk. The LCWC shall not be liable for any injuries, lost or stolen articles or damages to any property resulting from use of the facility.

11. Security: All partners and guests must enter and leave the facility via the main entrance.

12. Expulsion: Personnel of the LCWC have the right to deny privileges based on a partner’s behavior which is either potentially harmful or disruptive to self or others. A partner may be expelled for violation of any rule or any other conduct deemed detrimental to the general wellness of the LCWC. The partner’s initiation and monthly fees may therefore be forfeited.

13. Refunds: All fees paid are non-refundable.

14. Amendments: Amendments to these rules and regulations may be made by the management without advance notice.
